A varicocele is the enlargement of veins within the loose bag of skin that holds testicles which is also called the scrotum. A varicocele is just like varicose veins in the legs. Varicoceles are a major cause of reduced sperm production and decreased sperm quality and motility which may lead to male infertility. Although not all varicoceles affect sperm production, varicoceles can sometimes cause the testicles to shrink.
A varicocele often produces no clear signs or symptoms. It might cause pain but in very rare cases. The pain may:
- Change from sharp to dull discomfort
- Increase with physical exertion or long time standing especially over long periods
- Worsen over the course of a day
- Be relieved when you lie on your back for a while
- Cause impaired fertility
With time, varicoceles might enlarge and symptoms become clearer. A varicocele has been described as looking like a "bag of worms." The condition might cause a swollen or reddish testicle, almost always on the left side.
To effectively manage a varicocele requires two main things. First, a healthy lifestyle to prevent the varicocele from increasing more and second, it is necessary to get rid of free radicals and potential toxins that have accumulated and cause the valves and arteries to function improperly by blocking the blood flow and causing inflammation.
Does a Varicocele lead to infertility?
The arterial and venous system brings and sends blood to and from the scrotum. This blood has to be free of any impurities and healthy to produce sperm. When the venous system fails to send the blood back due to a problem such as varicocele, there is more de-oxygenated blood in the testis. This can impact sperm production and quality negatively. Sperm count can decrease, eventually causing infertility in the long term. You can read about this in detail in this article.
So how and why does Varicocele occur?
The veins send blood back to our hearts. Blood has to move upwards from the scrotal area to the heart. To make sure that the blood travels in the upward direction, the veins can only send blood one way. Modern lifestyle can cause a pH imbalance due to toxicity in the cell that accumulates in the blood. This altered pH and accumulated toxins can form destructive molecules that can damage cells in your body, called free radicals. The accumulation of free radicals and toxins around the veins and valves in the scrotum leads to poor functioning and causes an obstruction in regular blood flow. Blood can start to accumulate in the veins to cause inflammation or a varicocele.
This can take up to months or even years for the veins to stop working properly and a varicocele to develop. This is mainly caused due to an improper lifestyle, which is why varicocele is considered a lifestyle disease.
How is a varicocele detected?
An ultrasound is performed to detect a varicocele. A sperm count analysis is also a good identification test in addition to the ultrasound for better judgment on how severe the disease is. There have been many reported cases where a varicocele is misunderstood as a hernia, epididymitis (inflammation of the tube in the testis) or a hydrocele (fluid collection in the scrotum causing inflammation). It is very important to get 100 percent accurate diagnosis.
Is Surgery Really the answer to varicocele?
According to modern sciences, varicocele treatment is only effective with a surgery. Modern science also indicates that varicoceles cannot truly be cured and has a recurrence rate of almost 35%. You can read the study here.
These are known as recurrent varicoceles. Note that a surgery does not ensure permanent relief and involves a recovery period of 4 to 5 weeks. On top of the varicocele, damage is done to the vas deferens which can cause a permanent reduction in sperm production. It is your decision whether surgery is right for you or not, especially after you are aware of the available non-surgical options.

The Secret to Treating Varicocele without Surgery
Surgery is definitely not a sure cure for varicocele. Surgery is a more high risk procedure with minimal benefits. Many natural treatment programs have gotten good enough that you do not need to consider surgery as an ultimate option anymore.
A few “Controllable” Risk Factors
Here is a short list of the controllable risk factors of varicocele.
- Smoking Cigarettes
- Drinking Alcohol
- Some Beverages
- Eating the Wrong Foods
- Postural Imbalances
- Poor Bowel Health (E.g. Constipation)
- Improper Underwear
- Excessive Sitting
- Wrong Exercise Type
- Unhealthy Masturbation
Does Smoking Cigarettes Cause Varicocele?
Yes, cigarette does contribute to both varicocele development and symptom severity. On estimation, about 30 to 35% of men who smoke develop varicocele (compared to 15% of normal population). Those who smoke also have a 5 times higher infertility rate than those who do not smoke at all and have more severe infertility symptoms.
This does not mean that if you do not smoke you cannot experience any varicocele symptoms. It just means that if you do smoke, you are more at risk to have varicocele and worse varicocele symptoms.
How is varicocele treated?
The decision to treat a varicocele depends on whether you are having symptoms and on whether you are trying to have children. Men who are not having symptoms usually do not need to be treated. For men who are having mild or occasional symptoms, the following steps may be enough to control the discomfort:
- Wearing a jockstrap during exercise or prolonged standing.
- Avoiding activity that causes the discomfort.
- Applying ice to the scrotum and groin, and/or.
- Taking the occasional over-the-counter pain reliever such as ibuprofen or acetaminophen.
If these non-invasive measures do not help relieve the varicocele symptoms, or if the man is worried about fertility, the varicocele can be treated with surgery or a procedure called an embolization. The goal of these treatments is to stop the flow of blood through the enlarged veins.
Some other important precautions
- Do not take hot baths or otherwise the pain and irritation will get worse.
- Do not do any heavy lifting if you are experiencing any pain symptoms. For at least a week during treatment, you should avoid any physical exertion or standing for any extended period of time.
- Do not go “all out” with exercise after getting better from treatment.
- After one to two weeks after the removal of the symptoms, you will be able to resume sexual activity. Please avoid any sexual activity to save yourself from any complication or pain.
